The North of Ireland Family History Society (NIFHS) has the following forms that are downloadable (if shown in italics) or can be completed online.
Family Group Sheet
Family Group Sheet — a PDF form for printing on which to record details of a couple and their children.
Member’s Profile Form
Use Member’s Profile Form (Word for editing) or Member’s Profile Form (PDF for printing) to list research interests or particular queries for publication on our website and in our eNewsletter supplement. Send it as a Word document or a scanned PDF document to ProfileForms@nifhs.org.
Relationship Chart
Relationship Chart — a PDF chart showing the relationship, e.g. degree of cousinhood, between two people.
Record of Ancestors Chart
Record of Ancestors Chart (Word) or Record of Ancestors Chart (PDF for printing) — a blank form on which to record several generations of your ancestors.
Individual Research Sheet
Individual Research Sheet — a PDF form for printing on which to record details of a single person.
Research Calendar
Research Calendar — a PDF form for printing on which to record the records searched or to be searched.
Birth Record Details Form
Birth Form — a PDF form for printing on which to record the registration details of a birth.
Marriage Certificate Details Form
Marriage Form — a PDF form for printing on which to record the registration details of a marriage.
Death Certificate Details Form
Death Form — a PDF form for printing on which to record the registration details of a death.
Society Membership Form
Please note that the next membership term is from 1st September 2024 to 31st August 2025.
However, new members are welcome to join “early” before 1st September.
The recommended way for joining or renewing membership is to use our Membership website at membership.nifhs.org
This is the quickest way as it avoids delay in receiving and processing an application.
Payment is made online using a recognised credit/debit card.
If a hardcopy form is needed for a postal application, please download the PDF Form.
Payment is made by sterling cheque.
Our Volunteers aim to complete the processing within 10 days of receiving a postal application.
